All forms for the 2012 Show Season are available on-line in interactive format. You can simply fill in the information on line, print, sign, and mail the completed form. If you would like to download the forms to your computer, download Foxit Reader (free), and then save the forms to your computer. This allows you to fill in all required fields once, then make any necessary changes to the form, and simply reprint the form. You will find this extremely helpfull, especially on entry forms. As an example, if you are riding the same horse in the same classes at all shows, all you have to change is the show date and reprint the form. This is just one way that we are trying to make horse showing a more pleasant experience.

2012 Stall Reservations

All season stall reservations are available first come, first served, and have to be paid in full at the time of reservation.  We will offer first right of refusal to barns that reserved season stalls in 2011, through March 1st.   

To reserve a block of stalls, download and print the 2012 Block Stall Reservation Form.  This form should only be used to purchase a block of stalls for the season.  If you intend to purchase stalls show-to-show, or you only need to reserve one stall for the series, use the Stabling Emergency Contact and Release Form.

All horses or ponies stabled at The Prince Georges Equestrian Center during a BEST Horse Show need to have a Stabling Emergency Contact and Release Form completed and filed with the Horse Show Secretary.

BEST Horse Shows Membership Form (Updated for 2012)

In order to be considered for BEST Horse Show Series awards, both the rider and horse need to be registered members of Black-Eyed Susan (BEST) Horse Show Series. Download and print the BEST Horse Shows Series Membership Form here.

Maryland Horse Shows Association, Inc (MHSA) Membership Form
